How do I obtain my $200.00 credit card discount for opening a Breeze Visa? I booked a flight and the discount didn't appear.

Can a 15 year old with no ID card fly by her self from Las Vegas to San Bermardino and return if her parents and/or grand parents drop her off and pick her up at the air port?
